- Employer with one or more employees.
- Employee elects to have a set dollar amount up to the government established limit deducted pre-tax from their paycheck to pay for out-of-pocket medical expenses.

BASE® 125 Limited Purpose FSA |
- Employer with one or more employees.
- Employee elects to have a set dollar amount up to the government established limit deducted pre-tax from their paycheck to pay for eligible dental and vision care expenses, including orthodontia.

BASE® 125 Dependent Care Assistance Plan (DCAP) |
- Employer with one or more employees.
- Employee elects to have a set dollar amount up to the government established limit deducted pre-tax from their paycheck to pay for child and/or dependent care expenses.

BASE® 125 Premium Only Plan (POP) |
- Employer that sponsors a group underwritten plan.
- Employees must pay a portion of the premium.
- Employee’s portion of premium payment is pre-tax through their payroll.
